Long-Standing Pain, Treated Without the Journey
Oyster Bay is the end of the line, and it is meant literally. The branch terminates here, the service is limited, and the roads in and out wind.
For a one-off appointment that is scenery. For persistent pain, where the honest answer is a course of care over weeks rather than a single visit, it is the reason the course does not get finished.
Chronic pain does not respond to a single heroic session. It responds to consistency — a plan followed over weeks, adjusted as symptoms move.
Anything that makes a visit expensive in time makes consistency less likely, and a long drive out and back on winding roads is expensive in time. People do not decide to stop; they miss one, then two, and the plan quietly ends.
The ground here is genuinely uneven and the older housing has full staircases rather than the half-flights of the postwar county.
For persistent knee, hip or back pain that matters in a specific way: going down a slope loads differently from going up one, and a body guarding a painful joint will find a way around the load rather than through it. That compensation is usually part of why the pain has stayed.
The practice comes to you, which for Oyster Bay removes the single biggest reason a course of care gets abandoned.
It also means the assessment happens on the hill you live on and the staircase you use, rather than on level flooring twenty minutes away. For pain that is being maintained by how you move, that is not a convenience — it is most of the diagnosis.

You will know the cost before anything is booked. We confirm a visit time that works around your day, explain how payment works, and tell you whether a physician referral will be needed for the length of care you are likely to need.

Follow-up visits happen at home, and the plan progresses as your tolerance and symptoms change.
There is no exact threshold, but pain lasting beyond the normal healing time for the tissue involved — often taken as around three months — tends to behave differently from a fresh injury. Progress may be slower. That is a reasonable goal rather than a lost cause.
Usually not. Long-standing pain is rarely a single damaged structure by that point; it is a pattern of loading, avoiding and compensating that has settled in. Patterns are changeable, and the fact that it has been there a while is a reason to assess how you are moving rather than a reason to stop.
It can. Walking downhill loads the quadriceps differently from walking up, and a joint that is being protected will offload to somewhere else. On flat ground that compensation can go unnoticed for years. On a grade it shows up quickly, which makes it easier to work on.
Sudden severe pain, chest pressure, new numbness or weakness, a fever, an injury after a fall, or any rapid change in function. Those need urgent or emergency care, not a physical therapy appointment, and we will tell you so on the first call.
No. You can begin with a direct evaluation.
New York's direct access provisions allow a licensed physical therapist to evaluate and treat you without a physician referral, for up to 10 visits or 30 days from your first treatment, whichever comes first. If your care should continue beyond that, we will tell you what is needed and help you arrange it.
If Medicare is paying for your care, your plan of care also needs to be signed by your physician, which we help arrange. Neither one holds up your first appointment.
Strength Works PT is credentialed with Medicare and Aetna. Plans differ, so call and we will check yours before anything is booked.
If your plan is not one we bill, or you would rather keep the plan out of it, you can pay for your care directly. Either way you know the cost before the first visit.
